Through a partnership with Welby Health, eligible patients may receive additional care support designed to help manage health conditions, stay on track with treatment plans, and feel more connected to their care team. This program does not replace your primary care doctor. Instead, it adds an extra layer of support so you have guidance and help when you need it. If you qualify, you may have access to a dedicated nurse care manager, personalized care coordination, and additional health monitoring tools that help your care team stay informed.
How This Program Supports Your Health
The goal of this program is simple. It helps patients feel supported and confident in managing their health outside of regular appointments. Through the partnership between North Coast Family Medical Group and Welby Health, eligible patients may receive additional care services such as:
- A dedicated nurse care manager
- Regular health check-ins and guidance
- Help coordinating care and appointments
- Personalized care planning
- Optional remote health monitoring when appropriate
These services help patients stay connected with their care team and address concerns earlier, often before they become bigger health issues.
A Dedicated Nurse Care Manager
One of the most valuable parts of this program is having a consistent point of contact for your care. If you participate, you will be paired with a nurse care manager who works closely with your medical team. Your nurse care manager can help answer questions, provide guidance, and check in regularly to make sure your care plan is working for you. They are there to support you, help you stay organized with your health goals, and make it easier to navigate your care.
Optional Remote Health Monitoring
For some patients, remote health monitoring may be recommended as part of their care plan. This allows certain health information to be safely tracked from home using simple devices. Monitoring important health indicators can help your care team recognize changes earlier and respond quickly when needed. Not every patient will need remote monitoring. If it is appropriate for your care, your team will explain how it works and answer any questions.
