These questionnaires are designed to screen young children, ages four months to five years old, for developmental delays. The purpose is to identify those children who are in need of further evaluation while also reassuring parents whose children are developing typically. This system represents a novel approach to screening because the questionnaires are designed to be completed by the parents or caregivers of young children, who spend the most significant amount of time with them and can accurately note their behavioral patterns.
These are designed to be used only in conjunction with an office visit and exclusively for the use of NCFMG patients. Please make every effort to complete these questionnaires prior to your visit so your provider can review your results.
